Located in Buena Vista, Colorado, Premier Home Care serves the communities

Registered
Nurses: Act as integrated case/disease managers overseeing
all aspects of the patient's plan of care from managing medications
to wound care. This provides you one point-of-contact for all aspects
of the patient's recovery process.

Certified
Nursing Assistants: Provide hygiene assistance, passive range
of motion, assist with transfers and ambulation until the patient
has sufficient strength to return to independent ADL.

Physical
Therapists: Work together to restore optimal strength, balance
and endurance.

|
|
Speech
Therapy: Our Doctor of Speech Therapy works to reduce the
impacts of a patient's impairment on swallowing and speech functions.

|
Medical
Social Worker: Provides social work to our area clientele
by providing an avenue of counsel to caregivers experiencing stressors
when caring for loved ones. |
At Premier Home Care, our goal is to help ensure your dignity,
recovery, comfort and independence. To achieve that goal,
we will work with you, your doctor, and your family members to create
a treatment plan tailored to their unique, individual needs.
You
may need home health care without prior hospitalization, but should
you be hospitalized, the discharge planner will list us on a choice
list. You may choose the agency you desire. Please remember, you are
not bound to use a hospital-based agency.
